Job overview We have an exciting opportunity for a Nurse Practitioner to join us in the Urgent Community Response/Virtual Ward Team.
The role involves supporting the delivery of the 2 hour Urgent Community Response and Virtual Ward mandates to improve out of hospital care and support the management of patients in their own home.
The team provides urgent care to people in their homes, helping to avoid hospital admissions and enabling people to live independently for longer.
Main duties of the job
Assess, plan, implement and review patients in their own home using research and evidence-based knowledge.
Monitor and improve standards of care through supervision of practice, clinical audit, and evidence-based practice.
Person specification Education and training
1st level registered nurse
Degree qualification or equivalent experience
Prescribing qualification or ability to work towards it
Teaching/mentoring qualification
Urgent care/ED experience or additional qualification
CSP or equivalent
Knowledge
Broad range of specialist knowledge of medical conditions and treatments
Knowledge of relevant Government legislation and health economy pressures
Evidence of CPD
Knowledge of Trust policies and procedures
Experience
Experience of managing a team/day-to-day workload
Evidence of managing change
Evidence of autonomous practice
Experience of responding to sudden and unplanned changes in medical conditions
