Stockport NHS Foundation Trust We are looking for a dynamic, enthusiastic practitioner to support the delivery of the 2 hour Urgent Community Response and Virtual Ward mandates to improve out of hospital care and support the management of patients in their own home. The team provides urgent care to people in their homes which helps to avoid hospital admissions and enable people to live independently for longer. Job overview

We have an exciting opportunity for a Nurse Practitioner to join us in the Urgent Community Response/Virtual Ward Team. Main duties of the job

Assess, plan, implement and review patients in their own home using research and evidence-based knowledge. Monitor and improve standards of care through supervision of practice and clinical audit. Person specification

Education and training

1st level registered nurse Degree qualification or experience to demonstrate level of working Prescribing qualification or ability to work towards and advanced clinical skills qualification or equivalent Teaching mentoring qualification Urgent care/ED experience or additional qualification Knowledge

Broad range of specialist knowledge of medical conditions and treatments Knowledge of relevant Government legislation and health economy pressures Evidence of CPD Experience

Experience of managing a team/day to day workload Evidence of managing change Experience of responding to sudden and unplanned changes in medical conditions Community experience/crisis response Skills and abilities

Excellent communication and interpersonal skills Able to work under pressure Effective written and oral communication Work related circumstances
