The post holder is responsible for the delivery of Respiratory Diagnostictest within the Community Diagnostic Centre Skegness. The focus of the role is both thedelivery of evidence-based Community care for patients who are symptomatic/presentinga respiratory Condition, and the provision of preventative health care to the CommunityDiagnostic Centre population. As a Respiratory Community Diagnostic Centre Nurse,you are responsible for the care delivered. The community Diagnostic Nurse will be required to carry ourspirometry and Feno testing on patients that have been referred to thecommunity Diagnostic centre. You will work collaboratively within thegeneral Community Diagnostic Centre team to meet the needs of patients, supportthe delivery of policy and procedures, and provide nurse support as required. The postholder should work within the NMC code of conduct and ensure othernon-regulated team members work within their code of conduct. Main duties of the job

Assess, plan, implement and evaluate treatment programmes that promote health and well-being Proactively identify, manage treatment plans for patients at risk of developing a long-term condition as appropriate. Work with other health care professionals to diagnose, monitor, manage and treat long-term conditions, including non-drug-based treatment methods using a management plan, and in line with national and local policies. Work with patients to support adherence to prescribed treatments. Support patients to adopt health promotion strategies that promote patients to live healthily and apply principles of self-care.
