An exciting opportunity has arisen for you to join our dynamic Community Children's Nursing team, where you will support your nursing colleagues to make a difference and care for children and their families within their own homes following discharge from hospital or receiving long term care. You will have the opportunity to learn new skills and increase your knowledge and skill base. You will work as part of a team, being able to prioritise your own workload, covering a large geographical area, delivering care in both a creative and empathetic manner.

To be able to assess and develop plans of care to meet the needs of patients. This will include continuously evaluating and acting on outcomes.

To provide patients and relatives with information and education. To undertake home visits to service users. To maintain confidentiality. Perform clinical observations, be able to interpret them and escalate when necessary. To download sleep study machines (training provided). To perform peripheral blood sampling (training and competencies provided). To pass nasogastric tubes (training and competencies provided). To supervise pre-reg nursing students. To be responsible for organising and prioritising own workload in the day to day allocation of work. To identify and escalate safeguarding concerns in accordance with local procedures. Monitor the condition and health needs of patients ensuring appropriate escalation to a Registered Nurse when required. Communicate effectively with dignity and respect to all patients, relatives and others.

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focussing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

Probationary Period

This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

High Cost Area Supplement - Fringe

This post also attracts payment for High Cost Area Supplement of 5% of the basic salary (with a minimum of £1,258 to a maximum of £2,122 p.a. pro rata for part time).

Qualifications and Responsibilities

The post holder will have the NNEB, CACHE or equivalent qualification and robust child development knowledge. They will have experience working with children with different needs. The post holder will support their peers and team leader and be an effective team member whilst working on their own initiative. They will have a commitment to collaborative working and actively support and liaise with other health and social care professionals and agencies. The post holder will be responsible for patients allocated to them and will observe the child in their home environment and/or school/nursery, identifying the child and family’s need and providing support to children and families/carers. The post holder will support families by signposting children/YP, families/carers to available resources within the borough for additional support and information. The post holder will assist the team members and the wider multi-disciplinary team to contribute to the management/assessment of children/young people (YP). The post holder will be an advocate for children and families within the service. The post holder will establish and maintain effective communication with children, young people, parents and carers, colleagues, multidisciplinary team, and Local Authority to achieve a positive health status for the child and family.
