Are you passionate about providing high quality patient care? The Liskeard-Looe Community Nursing Team are looking for motivated practitioners to join us. You should possess commitment to team working and innovation in the delivery of excellent standards of care. This Nursing Associate in the role needs to demonstrate excellent organisational skills. People approaching the end of their lives who have been referred to the team may need to be seen urgently and the person in this role will need to help to coordinate the Health Care Assistants and other resources to deliver this urgently needed care. The successful candidate will be delivering high quality care and will also have a key role in supervising Health Care Assistants and delegating appropriate work to them. The Nursing Associate will also assist the team in coaching and teaching the Health Care Assistants and other members of the team. This is an exciting and fulfilling role and you will need to be able to demonstrate a real enthusiasm for nursing people in the community especially those approaching the end of their lives as well as a desire for on-going professional development. You should also have an understanding of the changing climate of care delivery in the community with the ability to travel independently across the area to fulfil the requirements being essential. Main duties of the job

The role of the Nursing Associate is to work with the Registered Community Nurses and other members of the multi-professional team, providing and monitoring care and contributing to the ongoing assessment, as agreed with supervising clinician(s) and in line with the service pathways. They will monitor the condition and health needs of people within their care on a continual basis in partnership with the multi professional team. This role will support the Community Nursing team with organising the patients on the end-of-life or Continuing Care at Home caseload. Support the team with coordinating and delegating work to the Health Care Assistants. Support the team with organising other resources such as equipment. Support the team by providing supervision, teaching and coaching. To review patients on the case load and adjust care plans. Working for our organisation
