Our Service The Community Health & Social Care team cover a large area including some rural communities as well as towns and villages across North Devon. Our team is closely connected with NHS teams, other social care teams, district councils, housing, and equipment providers. We are an enthusiastic supportive team made up of social care assessors and both band 5 and 6 occupational therapists. The team occasionally carry out assessments over the telephone or virtually using other forms of technology but the majority of our work is completed face to face in the person’s own home. About the role You will be part of a friendly, supportive, and experienced team with regular peer support, supervision, appraisal, team meetings, and a commitment to learning and development, having access to regular CPD sessions. You will have the opportunity to travel around the very scenic Torridge area of Northern Devon to assess and support the people that we work with. You will be assessing the needs of adults with a wide range of health conditions including physical disability and learning disabilities across the Torridge area. Clinical responsibilities will include enabling people to maximise their independence and potential, as well as increasing their safety through reducing and managing risk to themselves and their carers. You will be required to undertake moving and handling assessments and reviews. In Devon, we utilise an optimal handed care focus through our moving and handling process. You will also undertake housing assessments and make recommendations on both minor and major adaptations, working alongside our equipment suppliers and local district councils in the delivery of disabled facilities grants. You will identify where equipment and technology can be used to optimise a person's independence and safety. As part of your assessment process, you will also consider preventative measures and safeguarding, as well as enabling a person to achieve their goals and full potential. You will join the locality Occupational Therapy duty rota providing timely interventions as required and support the team manager in triage of the Occupational Therapy waiting list. You will have autonomy in your role as well as opportunities for hybrid working. You will have support from both immediate team members and opportunities for joint working alongside our NHS rehabilitation colleagues and the specialist learning disabilities services, as well as wider social care teams, NHS, GP, Mental health, and voluntary sector services. As a wider team, we encourage the support of student placement, and there will be times when you are called upon to host (either individually or as part of the team) an Occupational Therapy student. About you You will be required to hold and manage a caseload and provide a person-centred service through clinical assessment within the context of local government policy and legislation.
