This is a 37h position but job share can be considered. Work alongside a dedicated and energetic team of Reablement Assistants and Occupational Therapists, providing therapy-led services to those in need and collaborating with a broader multidisciplinary team, including local hospital discharge teams. Together, you’ll create therapy pathways that maximise individuals’ independence. A valid licence to drive in the UK is required as the role involves travelling locally and lone working. Your Responsibilities Will Include: Supporting people with their personal care and empowering them to do as much as possible on their own. Monitoring and providing feedback on the progress of people’s goals. Assisting with medication and encouraging independence. Completing care records and other relevant paperwork. Strong interpersonal, communication, and ICT skills. Knowledge:

Health and Social Care legislation, safeguarding adults. Requirements:
