The Care of the Elderly wards at UHCW NHS Trust provide acute care for older adults, often who have multiple co-morbidity. The team specialise in supporting patients who have dementia, delirium, high frailty scores or may be deconditioned, and work holistically to provide comprehensive assessments and treatments which promote timely discharges from the Acute Trust.
We are looking for Occupational Therapists with an interest in acute therapy and experience in supporting individuals with complex cognitive needs.
As a Band 6 into a new role, you will be supernumerary for 2 weeks. This will allow you to get to know the team, service and Trust and highlight any training needs.
Responsibilities: Manage a caseload of patients with varying diagnosis and/or complex needs, using evidence-based practice with client-centred principles, to assess, plan, implement and evaluate interventions. Support the learning and development of students and staff. Develop enhanced clinical skills and knowledge in an area of interest linked with patient cohorts (e.g., cognition, specific clinical condition, prehabilitation/acute rehabilitation). Participate in 7-day working patterns as required. Carry out treatment, using a broad variety of modalities and clinical reasoning to allow selection of the most appropriate technique evaluating, modifying, and recording all interventions and outcomes & adapting them to meet the needs of the patient. Use highly developed manual treatment skills requiring coordination, sensation & dexterity. Work in accordance with RCOT & HCPC rules of professional conduct and within local care pathways, policies and standards. Establish and maintain effective communication networks with patients, carers, MDT, other workers and agencies across health, social care and the private sector locally and regionally. Contribute to the Trust and Group clinical governance arrangements and quality agenda, including the setting and monitoring of practice standards, clinical risks. Take the lead for information management within area of responsibility to ensure high quality up to date written and electronic (where available) records and activity data are maintained in accordance with Professional and Trust standards & produce in a timely manner.
