What you will do: Lead assessments and create personalised care plans for individuals with complex mental health issues, ensuring pathways to support recovery and well-being. Work collaboratively with a multidisciplinary team to deliver comprehensive mental health services. Engage in risk assessment and management, considering the wider impacts of health and social needs. Manage a diverse caseload of service users, offering both base and community-based support, making a driving licence essential.

Requirements: Must have HCPC registration. A minimum of two years of experience in a similar role is essential. Proven experience in working clinically with service users experiencing a range of mental health problems. Ability to assess, plan, implement and review care needs alongside a multidisciplinary team. Strong understanding and experience in risk assessment and management.
