Be primarily responsible for the rapid assessment and treatment of patients who have urgent and/or complex physical/mental health and social care needs. Provide health care advice and education to team members, statutory, voluntary, and independent sector providers. Be responsible for health promotion, teaching, and training of support workers, pre-registration students, patients, relatives, and carers. Ensure up-to-date written and electronic records and activity data are maintained in accordance with professional and trust standards. Prioritise caseload, with professional demands, considering the needs of other members of the team. Person Specification
Experience
Community experience. Experience in discharge planning of patients from hospitals. Experience of caseload management and prioritisation. In addition to experience, all qualified roles will be required to provide evidence of registration with the relevant Registration Body. Qualifications
Current registration with HCPC. Disclosure and Barring Service Check
