The Role: The OT role is within the Integrated care team (LAP) which consists of Community Matrons, Mental Health Practitioners, Social Workers, Occupational Therapy Assistants, and Social Care Practitioners. The LAP team involves supporting people with multidisciplinary support to look at the needs of the resident in a holistic way helping to prevent longer term challenges and admissions to hospital.
