Experienceof Assessment and Treatment delivery in long term conditions and activemanagement. A highlevel of competence with IT including in Patient Management Systems such asEMIS Web. RiskAssessment Experience. Experienceof writing comprehensive clinical notes. Abilityto communicate effectively in both orally and in writing. Abilityto develop and maintain constructive relationships. Awarenessof the issues surrounding vulnerable groups and the boundaries of professionalrelationships. The role will involve working across any of our 5 local practicesand travel to complete home visits with patients across the locality and so afull driving licence and vehicle for work purposes is required. 37.5 hours per week inclusive of one day per week to attend University orcomplete online study to complete the Roadmap. Main duties of the job

Thispost requires an Occupational Therapist who is highly specialised and can workat an advanced level of practice within Leek and Biddulph Primary Care Network(PCN). The PCN's occupational therapyservice is separate from, but complementary to, the occupational therapyservices currently available within secondary services community therapy andsocial services OT. The remit of the PCN OT service is Early Intervention forindividuals with physical or mental health conditions. The service provides assessmentand intervention programmes for Pain Management, Fatigue Management, andCognitive Impairment. Environmental assessments within the home are completedas appropriate. The post holder will demonstrate safe, clinical decision-makingand expert care for patients within the PCN. They will need to be autonomousand carry their own clinical caseload under the supervision of the FCP OTs.Experience within community services essential. Upon completion of the roadmap the salary would increase. About us

We are amulti-disciplinary team, covering Leek and Biddulph, responsible for thehealthcare of just over 50,000 patients.We are led by Dr Neil Briscoe our PCN Clinical Director and a supportivemanagement team. We are a veryforward thinking and innovative PCN who were the first PCN in NorthStaffordshire to convert to a Limited Company.We utilise to the full, the skills and experience of our team memberswhich includes Clinical Pharmacists, Pharmacy Technicians, OccupationalTherapists, Physiotherapists, Social Prescribers and Mental Health Team. Wehave a flexible approach to working patterns and generous terms and conditionsincluding the NHS Pension. We aresupportive of professional development and pride ourselves on developing newroles in a collaborative and friendly environment. This is a permanentrole. InfectionControl All staffhave both duty and responsibility to avoid and prevent the spread of infectionas a consequence of any actions or activities they are carrying out during thecourse of their duties. The postholder must Take all reasonable care tominimise the risk to service users, colleagues and members of the public(including themselves) of healthcare-associated infections. Co-operate with the PCN inensuring that all statutory and mandatory requirements are complied with- inparticular, compliance with the Health and Social Care Act 2008: Code ofPractice for the prevention and control of healthcare-associated infections. Be knowledgeable of and compliantwith, the PCNs infection control policies and procedures, which includesattendance at mandatory training. DataProtection It is arequirement of employment within Leek and Biddulph PCN that all staff mustcomply with the obligation of confidentiality to personal identifiableinformation. The Data Protection Act 1998 safeguards the handling ofinformation held in both electronic and manual filing systems and it is theduty of all staff employed by the PCN to uphold the principles of the Act. Freedom ofInformation Act The Freedomof Information Act 2000 requires all public organisations to provide any memberof the public, upon receipt of a written request, with the information theyrequire, subject to certain exemptions. Every member of staff employed by Leekand Biddulph PCN must take responsibility, in conjunction with the PCN Manager,for any request for information that they receive and respond in line with thePCN Freedom of Information Policy and guidance. An ability to communicate effectively in the written and oral form and the understanding of how to develop and maintain constructive relationships Knowledge and experience of a variety of standardised and non-standardised assessment methods for a wide range of health concerns e.g. anxiety, dementia, stress, pain, fatigue, falls etc. Knowledge and skills in environmental assessment and provision of minor adaptation and equipment to support independence in ADLs around the home Ability to be self-motivated and work co-operatively as part of a team Knowledge and awareness of a variety of physical and mental health conditions and their impact on function. Knowledge of the role of OT within Primary Care and the evidence base that supports this. An ability to manage conflicting priorities and ones own time with a minimum of supervision to meet agreed deadlines Demonstrate a commitment to respecting and valuing Patients perspectives and choices. Motivated to work towards creating a safe, open and trusting environment. Awareness of the issues around working with vulnerable groups and the boundaries of professional relationships. Qualifications

BSc in Occupational Therapy under a training programme approved by the College of OTs Health & Care Professions Council (HCPC) registration Willingness to complete the FCP Roadmap or already hold the FCP Roadmap Post Grad Training within areas of Mental Health, Frailty, Dementia or Pain Management. Member of the Royal College of Occupational Therapy and or Specialist Sections. Experience

Experience in managing a designated caseload using theoretical and practical experience, completing assessments, planning, implementing interventions, and evaluating outcomes aligned to care plans Experience of assessment and treatment delivery in long term conditions and active management IT literate, including competency in Patient Management Systems (e.g. EMIS Web) and MS Office applications Extensive experience of writing comprehensive clinical notes, implementing and evaluating interventions that are evidence based but person centred. Experience of using Medequip services Community and lone working experience. Experience within both mental health and physical health settings. Experience of working within a primary care setting.
