What you will do:

Provide assessment and treatment for patients within their homes or suitable community settings, focusing on neuro and stroke rehabilitation. Develop and implement personalised treatment plans to aid recovery and maximise independence. Work collaboratively with multidisciplinary teams to ensure holistic patient care. Keep clear and concise records of patient progress, noting improvements and areas needing further intervention. Participate in team meetings and contribute to case discussions for holistic care planning.

Requirements: You need to have a minimum of one year's UK or NHS experience in a similar role. Being a car driver is essential due to the community-based nature of the role. An HCPC registration is required, ensuring you meet professional standards and are recognised to practise in the UK.
