Job Purpose:

To contribute to a high-quality service by carrying out assessment, goal setting, implementing treatment programmes, and evaluation of adult clients who may have complex and/or chronic presentations including musculoskeletal, sensory impairment, medical and mental health needs, with support and supervision from senior physiotherapists. To work within the Community Beds Team within the Health Reablement service. This may extend or change to other community services in-line with organisational development. To support clients to improve or maintain their level of function and quality of life and plan future care in an appropriate setting in partnership with multi-disciplinary team (MDT) colleagues, including liaising, supporting, and referring on to health services, voluntary services, and public sector agencies.

To participate actively in the services training strategy, including in-service training, organisation-specific and multi-agency training. To be responsible for planning a safe discharge from the service. To act as Key Worker as required.

Dimensions:

Responsible for Occupational Therapy assessment and treatment for a defined clinical caseload appropriate to the service area. Working in conjunction with and under the supervision of a senior OT and other senior clinical professionals. Lead group sessions where appropriate. Able to fulfil the physical demands of the role, including manual handling applications. Will be required to travel city-wide for client visits, meetings, and training.

To undertake day-to-day clinical prioritisation and work planning, supported by senior OTs and other clinical professions in the team. Leadership:

To establish positive and mutually supportive relationships with all team members. To promote the roles and skills of OT staff to other professionals. To contribute constructively to multi-professional team working. To demonstrate a can-do attitude and support others to do the same. Clinical Skills:

To be professionally and legally responsible and accountable for all aspects of your own work including the management of clients in your care. To ensure a high standard of clinical care for the clients under your management, and support other team members to do likewise, implementing appropriate teaching, supervision, and delegation skills where appropriate. To assess for, provide, demonstrate, and fit adaptive equipment using problem-solving and analytical skills. To accept clinical responsibility for a designated caseload of clients, and to organise this effectively with regard to clinical priorities and use of time and ability to be flexible and respond at short notice to the needs of the service. To conduct and report on clinical risk assessments as and when appropriate. Professional:

To undertake the comprehensive assessment of clients, including those with a complex presentation using investigative and analytical skills through standardised and non-standardised assessments. Formulate individualised treatment plans with rehabilitation goals using clinical reasoning in consultation with senior therapists as required. To advocate on behalf of clients and carers, where appropriate, overcoming barriers to communication such as cultural or language difficulties, physical or mental health difficulties, using skills of empathy to facilitate or empower the client. To complete patient/client records in line with organisation, regulatory body, and legal requirements. To demonstrate a commitment to evidence-based practice and the use of outcome measures and make recommendations for change. To maintain a high level of professional standards and work to the Professional Values and Behaviours as set by the OT professional body.

Planning and Organisation:

To be responsible for own time and caseload management. To liaise effectively with other professionals and agencies to enhance rehabilitation within the boundaries of Nottingham CityCare Partnership CIC and Social Care.
