The Community Rehabilitation and ESD Service are looking to appoint a dynamic, enthusiastic, and forward-thinking Occupational Therapist to join our service. The role will involve assessing patients, planning treatment plans, and coordinating support and equipment to meet the patients’ needs, working across both rehabilitation and ESD. Promoting continuity of care into the community setting and also working with the wider MDT to support long-term rehabilitation pathways for patients in their own homes to support, increase, or maintain their independence. We are looking for a therapist with excellent communication skills who can demonstrate sound clinical assessment skills and has the ability to work autonomously as well as part of a team. The role will involve a holistic approach to encompassing physical rehabilitation and referring to wider community services. You will improve the experience for patients and their families, promoting continuity of care for patients in the community setting, supporting patients following falls, and providing support for the Integrated Neighbourhood Team (INT).
