As a Band 6 Occupational Therapist , you will play a critical role in the Discharge to Assess multidisciplinary team. We will consider applications for people seeking full and part-time posts and flexible hours.
Your responsibilities will include:
Working with the wider MDT to provide specialist Occupational Therapy assessments and interventions.
Providing a specialist, high-quality therapy service within the Discharge to Assess Team.
Conducting Occupational Therapy assessment, diagnosis, treatment, and education for individuals with a range of conditions and presentations.
Offering advice, signposting, guidance, and information to health and social care professionals, patients, carers, relatives, or other non-professional contacts.
Being an integral member of the MDT, providing education, training, and dissemination of information to all healthcare professionals.
Acting as an autonomous practitioner, holding a caseload and working without direct supervision.
Continuing your professional development in line with HCPC regulations.
