This is a permanent role within the DTA Pathway. Livewell Southwest is one of the largest independent social enterprises in England. Livewell Southwest is currently looking for 2 Band 6 Physiotherapists within its Discharge to Assess Pathway. Working alongside Physiotherapists, Social Workers, Nurses, and Community Care Workers, you will be required to undertake functional and statutory assessments on behalf of Livewell SW. Livewell Southwest is fully committed to an integrated health and social care workforce that operates a shared skills model while adhering to the core principles of the therapeutic role. As a Band 6 Occupational Therapist, you will play a critical role in the Discharge to Assess multidisciplinary team. We will consider applications for people seeking full and part-time posts and flexible hours. Work with the wider MDT to provide specialist Occupational Therapy assessments and interventions. Provide a specialist, high-quality therapy service within the Discharge to Assess Team. Provide specialist Occupational Therapy assessment, diagnosis, treatment, and education to individuals who may have a range of conditions and presentations. Provide advice, signposting, guidance, and information to health and social care professionals, patients, carers, relatives, or other non-professional contacts. Be an integral member of the MDT providing education, training, and dissemination of information to all healthcare professionals. Be an autonomous practitioner, holding a caseload and working without direct supervision. Continue own professional development in line with HCPC regulations. Person Specification

Experience

Experience of working flexibly within a MDT. Clinical experience in health or social care / experience in OT treatment planning. Experience of multi-pathology assessment / home and community assessment. Experience of effective caseload management and discharge planning. Experience of management, leadership, and supervisory skills. Ability to work autonomously and set own priorities. Documented evidence of CPD. Experience of working with staff from other agencies. Experience of supervision of junior staff and/or assistants. Experience of written risk assessment in a variety of settings. Experience of standard prescription wheelchairs / static seating and pressure care. Experience of posture and positioning. Knowledge and use of complex manual handling planning techniques including written manual handling plans. Experience of innovation and supporting service development projects. Qualifications

BSc (Hons) or equivalent Occupational Therapy qualification. State Registered Occupational Therapist with Health and Care Professions Council Registration. Evidence of ongoing education / professional development. Clinical educator. Recognised qualification in written and spoken English if this is not first language.
