Occupational Therapist Team Lead (closing date: 10/02/25)

£44,711 - £48,710 per year. The focus of the service is to support community Reablement, prevent unnecessary admissions to hospital or long-term care, and facilitate timely discharge from hospital. The post holder will work in an Integrated Multidisciplinary Team with Health and Social Care colleagues. The service is provided to people over the age of 18 years residing within the City & County of Swansea. Assessments and interventions are completed within a wide variety of settings; peoples' homes, nursing & residential care facilities, residential and community hospital Reablement beds and In-reach across acute hospital sites. The service also accepts referrals for children who may require adaptations to their homes following OT assessment via Disabled Facilities Grants. The Occupational Therapy Team are present at the Common Access Point for Health and Social Care, contributing to the multidisciplinary screening of incoming referrals and carrying a complex caseload of crisis and urgent referrals. The team also participates in the Integrated Discharge Hub as part of the MDT screening to identify the correct pathway for hospital discharges. Main responsibilities: Evidence-based / client-centred principles to assess, plan, implement and evaluate the outcomes of intervention in hospital and in the community, maintaining associated records and organizing this efficiently and effectively with regard to clinical priorities and use of time. Provide highly developed expertise within the community setting. Provide clinical leadership for staff through supervision and appraisal as delegated by the line manager. Participate in the planning, development and evaluation of Therapy Services within the Integrated Community Hub, holding responsibility for defined projects. Be jointly responsible for the operational management of therapy teams within the hub, including caseload allocation, management, and professional development. Contribute to the maintenance and development of Therapy Services in the Community Hub setting and support a consistent approach to therapy provision across the locality. The successful applicant will be required to travel in a timely manner around the community and participate in a seven-day working roster. The applicant should be highly motivated, enthusiastic, flexible and resilient. Previous experience working in a Local Authority with caseload experience in Paediatrics and/or Adult is desirable. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
