Job summary Exciting opportunity for a band 7 Occupational therapy lead position within the MDT environment of the Urgent Care Team. Managing a team to support early hospital discharges and responding to urgent community response referrals to help prevent admissions to hospital. To lead the multi-disciplinary Team in providing an Urgent Care Response Service to patients to promote faster recovery from illness, prevent unnecessary acute hospital admissions, support timely discharge and maximise independent living. Working towards ensuring the national targets for urgent care response are met by providing timely interventions in line with the 2 hour and 2-day response times. Main duties of the job The main duties of this role will be to manage and clinically work within the urgent care team, predominantly in the Paignton and Brixham locality however may include Baywide working if required. Completing assessments within patients' homes, providing clinical supervision and support to the MDT, completing staff appraisals and supporting with the daily triage and prioritisation of new referrals linking in with the acute/community wards as well as GPs and community teams. The main focus will be around hospital avoidance rapid assessment to prevent hospital admission or facilitate early discharge. To include occasional weekend and bank holiday working to support the 7-day service between the hours of 8am and 7pm. Qualifications and Training Essential Occupational therapy degree HCPC, NMC or SW(England) registration Evidence of continued professional development Leadership experience within a health or social care setting Additional specialist training and qualification to post graduate diploma or equivalent in a relevant subject Knowledge and Experience Essential Good understanding of aims and priorities of Urgent Care Response. Significant clinical knowledge in area of own profession Proven ability of complex case management Extensive experience as a senior clinician/practitioner Evidence of effective staff and resource management Evidence of multi-disciplinary team working across agencies Specific Skills Essential Ability to deliver high quality services with good understanding of performance monitoring systems, quality measures and professional governance arrangements Strong interpersonal skills Excellent communication skills Leadership skills and excellent decision-making capability Excellent IT skills Ability to work effectively across organisations, cultures and systems to integrate services Patient and quality focused Able to motivate and inspire others Ability to effectively communicate complex information within a range of settings Able to work under pressure and meet deadlines Excellent organisational skills Self-motivated and able to use own initiative Flexible approach to change Physical Skills Essential Daily work includes driving Lift and carry equipment (wheelchairs, health care equipment) and furniture on a frequent basis Moving and handling of patients on a frequent basis in relation to assessment, treatment and rehabilitation. Treatment may necessitate restricted positions or limited space Work in the community where appropriate equipment may not be available.
