What will your responsibilities be? You will conduct Care Act assessments for individuals discharged from hospital to step-down beds. Collaborating with a multidisciplinary team (MDT), you will support individuals in returning to their homes and communities. You will arrange necessary care and support to facilitate a smooth transition and perform Continuing Healthcare (CHC) assessments, mental capacity assessments, and functional assessments as needed.
