The States of Guernsey are looking for a Senior Mental Health Nurse Practitioner to join the team at the Older Adult Community Mental Health Team, which incorporates the Memory Assessment Service.

The successful candidate will hold a caseload of patients with a range of complex mental health difficulties. Responsibilities include: Providing assessment, treatment planning, risk assessment, and management for patients. Offering ongoing community support for families and carers. Ensuring that systems and processes are in place to provide integrated care pathways and seamless transitions between departments and services. Demonstrating knowledge, skills, and competence in undertaking memory assessments. Providing post-diagnostic support and signposting.
