We have an exciting opportunity to join the Oncology Team as a SACT Navigator within our Chemotherapy Suite.
You MUST have Oncology experience to apply for this post.
We are looking for somebody who is self driven, extremely organised, has excellent communication and interpersonal skills and who is confident to communicate with a variety of teams.
Your role will involve working alongside other health professionals such as the medical teams, pharmacy, pathology, nursing and the patient and their family/carers. You will also develop good working relationships with secondary care colleagues and community colleagues.
Main duties of the job
The Oncology SACT Navigator will form an integral part of the multidisciplinary team in caring for the physical, social, and psychological needs of patients and carers who attend the Chemotherapy Suite throughout their treatment pathway and will under the guidance and supervision of the Oncology Unit Manager/Unit Co-ordinator, be responsible for co-ordinating the patient pathway.
The post holder will have daily direct contact with patients and their families, providing support, advice, and will be the main point of contact liaising with the medical, nursing and pharmacy teams.
The role will also involve the undertaking of administrative tasks deemed suitable to the role by the Oncology Unit Manager.
Job responsibilities
Support the Oncology Unit Manager/Unit Co-ordinator by assisting the nursing team in coordinating the Unit's activities to maximize efficiency and prevent delays within the pathway. Liaise with the nursing and oncology pharmacy staff to coordinate the supply of systemic anti-cancer treatments (SACT) so that it is available for each patient's appointment. Liaise between nursing staff, aseptic pharmacy, and outpatient pharmacy (TTOs) regarding patient attendance and communicate go ahead or any delays. Triaging telephone calls and providing basic advice, support, and signposting. Liaising and referring to other appropriate healthcare professionals. Ensuring that any investigations are booked, and the results reviewed and authorized appropriately prior to patients receiving treatment. Ensure the appropriate management of patient information and the coordination of patient pathways through collaborative working with medical secretaries, MDT coordinators, clinical nurse specialists and other departments as required within the Trust. Ensure the appropriate and timely booking of follow-up outpatient appointments in conjunction with treatment appointments. Support the nursing and pharmacy team in ensuring patients receive their supportive medication prior to discharge. Refer patients to the community nursing team as directed by the nursing team. Data Collection and Analysis Collect data for the Oncology Unit as requested by the Unit Manager or the Lead Nurse. Quality Issues Review the Unit's treatment schedule (Mosaiq), ensuring that appointments are correctly booked, and capacity is maximized. Assist in the development of care pathways and implement improvements to the administrative element of the pathway. Identify areas where targets are not achieved and undertake process mapping to identify bottlenecks and improve performance. Professional Participate and display a willingness to learn and develop relevant technical competency/area of interest and keep up to date with current local and national issues and developments. Be responsible for own learning in knowledge base relevant to own clinical area including mandatory training. Participate in regular formal supervision and develop a personal development plan that links into KSF and departmental competency frameworks to promote learning and enhance skills. Manage own daily timetable of delegated tasks including time for supervision and training. Other responsibilities Undertake any such duties as may be required from time to time, that are consistent with the responsibilities of the grade. Comply with the Trust's and department's policies, guidelines, and procedures. Attend all mandatory training as required in line with the Trust employment and professional need. Participate in team meetings and the review and development of the SACT Service. Inform the line manager of any issues relating to the ability to safely carry out duties. Be aware of departmental duties and initiatives to ensure a safe environment, recognizing patients' requirements for privacy and dignity. Recognize and comply with the Trust and departmental policies and procedures for incident reporting. Ensure confidentiality in accordance with Trust policy and procedure.
