Halton OOH District Nursing service is seeking a committed and enthusiastic individual to join their team as a community staff nurse, covering the evening service.
The post is 18 hours per week over 3 evenings, hours are 17:00 - 23:00.
Applicants must have excellent interpersonal and communication skills, have a patient-centred approach to care, be highly motivated, and able to work autonomously as well as part of a team.
The district nursing service provides nursing care to the housebound population of Halton, to patients in their own homes as well as residential care homes. We also provide a treatment room service for ambulant patients and offer a comprehensive range of clinical skills.
You will work under the direction of the District Nursing Sister delivering: Administration of IV medication/Medicines management Health Promotion End of life/supportive care Holistic assessments Patient-centered Care Complex wound management Catheter care
Main duties of the job
The successful candidate will perform holistic assessments of patient nursing needs. Plan, assess, implement and evaluate care delivery according to changing health needs utilising prescribing skills.
Ensure practice is evidence-based through undertaking audits and critically appraising literature to achieve a high standard of care.
Liaise with the District Nurse team leader to ensure information sharing in relation to patient care/changes to care plan and the continuity of care and reviewing of complex care packages.
Mentoring, Teaching and Assessing.
Liaise with all health care professionals and other agencies to ensure effective service delivery.
Involve patients and carers in the planning and delivery of care.
Maintain accurate records in accordance with NMC Standards.
Participate in the health needs assessment of the practice population to identify areas of unmet need.
Ability to work without direct supervision and to solve problems.
Work on a rota basis to facilitate weekend working across the district nursing teams.

Ability to develop effective interpersonal relationships with colleagues across health and social care settings. Ability to communicate at a high level both verbally and in writing. Good IT skills.
