Outpatient Assistant Practitioner

The Outpatient Assistant Practitioner will work as part of a Cross site Team - working at Heald Green, Little Hulton, and other outreach settings. Hours

As the Outpatient Assistant Practitioner, you will play a crucial role in delivering high-quality palliative care to our patients. This non-occupational specific role has been developed to support our outpatient and outreach services. You will work autonomously within the clearly defined boundaries, without the need for daily supervision, taking responsibility for your tasks and supporting others within the care pathways. The Outpatient Assistant Practitioner will be an integral part of the multidisciplinary team developing and maintaining strong working relationships with internal colleagues and external referrers/stakeholders to support timely and efficient access to outpatient service provision and discharge from outpatient services. We are looking for someone who holds an Assistant Practitioner Qualification or Registered Nursing Associate Qualification. You should have excellent verbal and written communication skills, with experience in delivering health or social care in a similar role. If you have completed communication skills training, that would be an advantage. In this role, you’ll need to be proactive, using your initiative to manage your time and workload effectively, especially when faced with competing tasks. Basic IT skills, including proficiency in Word, Outlook, and Teams, are important. Additionally, we value someone who can identify their own development needs and demonstrate a commitment to continued professional development. You should have a solid understanding of specialist palliative care and be comfortable working both independently and as part of a team. Familiarity with Electronic Medical Record systems, such as EMIS Web, and the ability to update care records accurately is essential. Maintaining strict confidentiality is a must, as is working with patients in a kind, empathetic, and person-centered way, aligning with our clinical strategy.
