£25,397 - £30,991 per annum Sanctuary Personnel are looking for an Outreach Case Manager to join our client's dynamic team, providing vital support to individuals navigating substance misuse in their community. Engaging with individuals in various outreach settings, guiding them towards treatment services and community support. Conducting thorough assessments to understand individual needs and strengths. Delivering tailored care packages, including therapeutic interventions and psychosocial support. Collaborating with healthcare professionals and contributing to multi-disciplinary team meetings. Managing cases with a recovery-focused approach, utilising both one-on-one and group sessions. Providing sensitive communication to service users and their families. Contributing to safeguarding efforts and maintaining safety protocols. Compiling reports, participating in care planning meetings, and ensuring accurate record-keeping. Developing and delivering training sessions for internal and external stakeholders. Actively promoting harm reduction and encouraging access to support services.
Requirements:
Relevant degree or diploma in Social Work, Psychology, or Substance Misuse. Demonstrated experience in outreach or similar roles within substance misuse field. Strong knowledge of substance effects, mental health issues, and therapeutic interventions. Ability to work autonomously, manage time effectively, and collaborate with diverse teams. Excellent communication, organisational, and problem-solving skills. Empathetic, non-judgemental approach with a commitment to professional boundaries.
