This is an exciting opportunity to join the Specialist Palliative Care team and enhance your palliative care skills and expertise.

Delivering high quality palliative care to patients within the community setting, with the potential to rotate into the acute hospital setting.

You would ideally have some knowledge of symptom control for patients with end of life malignant or non-malignant care needs.

The job will involve the education and training of other health professionals regarding end of life and specialist palliative care across North West Anglia NHS Trust and the local community.

In supporting a seven-day specialist palliative care service, the successful candidates will need to work weekends and bank holidays pro rata.

The role offers an opportunity to develop palliative care knowledge and experience across the hospital and community provision in the region.

This secondment or fixed term opportunity does have some potential to become permanent in the future, though this is in no way guaranteed.

Main duties of the job To deliver high quality holistic palliative care interventions to patients with complex palliative care needs, their families, and support care teams. To demonstrate nurse leadership in action, role modelling Trust values whilst delivering patient-focused care. To deliver teaching formally and informally to patients, carers, and colleagues. To engage with service improvement tools, including audit to support service innovation. To develop skills and expertise and deliver plans of care delegated by the Specialist MDT.

Job description Job responsibilities Be an accountable professional acting in the best interests of people, putting them first and providing complex clinical care that is evidence-based, person-centred, safe, and compassionate. Delivering care planned and delegated by the Specialist Multidisciplinary team. Use existing knowledge and expertise and enhanced levels of clinical judgement to undertake complex and holistic assessments within the scope and level of practice expected of a Palliative Nurse. Act within the scope of practice and expertise to plan, deliver, monitor, and evaluate care using clinical assessments, diagnostics, and interventions. Act as a resource within their own organisation and for external agencies. Develop, deliver, and evaluate education and training opportunities for others within own scope of practice. Communicate effectively in challenging environments and situations with patients, their families/carers, and the multi-disciplinary team. Encourage and engage with innovative clinical practice to support a culture of excellence within the wider health and care team. Manage unpredictable and unplanned clinical situations with support from senior MDT colleagues. Participate in resource management, service development, planning, and service improvement. Develop and appraise staff and learners in line with expected level of competence.

Person Specification Education & Qualifications Essential Specialist module in palliative field, or working toward, or willing to achieve within agreed fixed time period. NMC or HCPC Registered Professional.

Desirable Advanced Communication Skills. Independent Nurse Prescribing. Masters level qualification or unit in specialist field.

Experience Essential Can demonstrate nurse leadership through extended roles. Multi-disciplinary Team working.

Desirable Up to date evidence of 4 Pillars of Practice. Managing people.
