Our Palliative Care Specialist Nurses work independently as part of the well-established community team at Ashgate Hospice, supporting patients and carers with palliative and end-of-life care needs across North Derbyshire and Derbyshire ICBs. The community team works closely with Ashgate multidisciplinary team and wider system to deliver patient-centred, evidence-based specialist services, ensuring high-quality care. The team consists of Palliative Care Specialist Nurses, Palliative Care Nurses, and Palliative Care Support Workers and are an integral part of Specialist Palliative care services at Ashgate Hospice. Key responsibilities: The Palliative Care Specialist Nurse will work independently as a member of the Community Palliative Care Specialist Nurse team across North Derbyshire based at Ashgate Hospice. The team provides patient centred, evidence-based specialist services for patients and their carers with palliative and end-of-life care needs, working where required across Derbyshire ICBs. In addition to the direct clinical role, the post includes leadership, managerial, educational, audit, and research responsibilities. The post holder will be expected to have a flexible approach to working which includes weekend and bank holidays. What you’ll bring: Here’s what we’re looking for: NMC registration, with post-registration extensive experience at Band 6 or above. Level 6 degree in palliative care or equivalent Advanced pain and symptom management CPD Advanced Communication Skills Training (or equivalent) Experience in relevant clinical settings such as palliative care, oncology, LTC, and Community.
