Ashgate Hospice, based in North Derbyshire, offers free palliative and end of life care for adults with life-limiting or incurable illnesses, including cancer, neurological diseases, and advanced heart, kidney, and lung conditions. Last year, we cared for over 2,400 people. Our aim is to support our patients by keeping them informed, managing their symptoms, and improving their quality of life from diagnosis onwards. We also provide vital support to the people who are important to them, offering practical assistance and emotional support, including bereavement services. We're more than a care provider. We're a campaigner for sustainable end of life care funding in the UK, and we continually invest in our services and workforce. Job responsibilities

Our Palliative Care Specialist Nurses work independently as part of the well-established community team at Ashgate Hospice, supporting patients and carers with palliative and end-of-life care needs across North Derbyshire and Derbyshire ICBs. The community team works closely with Ashgate multidisciplinary team and wider system to deliver patient-centred, evidence-based specialist services, ensuring high-quality care. The team consists of Palliative Care Specialist Nurses, Palliative Care Nurses and Palliative Care Support Workers and are an integral part of Specialist Palliative care services at Ashgate Hospice. The role involves providing patient-centred, evidence-based care and encompasses clinical duties, as well as leadership, management, education, audit, and research responsibilities. Flexibility is required, including availability for weekend and bank holiday shifts. Person Specification

Qualifications

Current registration on Part 1 of the NMC register Level 6/7 degree in palliative care (or related subject) or a commitment to undertake as part of Advanced Skills training (NMP/APPACS). Non-medical Prescribing or a commitment to undertake within a specified time frame Advanced Physical Assessment and consultation skills or a commitment to undertake within a specified time frame Evidence of continued professional development Advanced pain and symptom management CPD and a commitment to undertake regular training. Teaching and Assessing Qualification/Mentorship qualification or a willingness to undertake. Disclosure and Barring Service Check
