Working with rehabilitative support staff to assess and treat patients. Working with our community nursing team to ensure a smooth transfer of expertise between nursing and AHP staff. Supporting patients to have a "one stop shop" experience with other Hospice services. Communicating effectively with colleagues, patients and relatives. Running various groups including shortness of breath, fatigue, balance and drop in gym groups. Visiting the community when required. About us

Anyone living in Basildon and Thurrock can call on us 24 hours a day, seven days a week. Working alongside other healthcare providers, our specialist services are available at the hospice, out in the community or in a persons place of residence. Qualifications
Membership of CSP Membership of ACPOPC desirable but not essential. Experience
Willingness to learn about palliative care. You will need to have a valid UK drivers licence. Access to a car for community visits. Disclosure and Barring Service Check
