Working as a member of the clinical pharmacy team providing a safe and effective service to a variety of patients. The role is 80% ward based, patient facing. Responsibilities include: Taking an up to date accurate medication history for all patients, working with the multidisciplinary team to resolve queries. Ordering patients' own medication ensuring sufficient supplies are available. Being a Pharmacy representative delivering a core service, answering Pharmacy related questions or referring to the most appropriate colleague within the team. Supporting the dispensary workflow with a daily end checking slot. Supporting the operational team as and when required with a variety of tasks to ensure timely delivery of Pharmacy supplies.
