An exciting opportunity has arisen in the Leaving Care Team at Swindon Borough Council for a Pathway Advisor working with young people aged 16 to 25 who have been in our care, supporting them to live independent lives and ensuring they have the right skills and opportunities to lead successful lives as they become adults. We are a service that has high ambitions and aspirations for all our young people. We would be particularly interested to hear from candidates who have a background in working with adults who have been impacted by their emotional and mental health. Your next role

As a Pathway Advisor you will be involved in planning and delivering a range of activities, opportunities and services to young people including: Providing advice and support Drawing up the Pathway Plan and ensuring it addresses any changing needs Keeping in touch with the young person Co-ordinating services, linking in with other agencies Minimum Requirements: Candidates must be fluent in the English language (as a requirement of Part 7 of the Immigration Act – for the effective performance of a customer-facing role).
