Purpose of Job: Support optimal care; maintain and document quality care measures and report findings to monitor statistical outliers. Gather and manage information in a high quality, cost effective way in order to improve the health of our patients and populations, respond to the demands of the oversight agencies (public and private) while maximizing the incentives (minimizing the penalties) available by meeting the goals described by those oversight organizations. Essential Functions: Implement and maintain quality standards/guidelines necessary to achieve the recognitions, which benchmark primary care. Identify, educate, support and coach all team members on the proper documentation processes associated with the above mentioned recognitions. Coordinate optimal transitions of care through communication with the patient, caregivers, clinicians, facilities, etc. in accordance with policies, guidelines, treatment plans and standing orders. Communicate with and schedule appointments according to care standards using guidelines to identify and close care gaps, provide support and education on self-management of chronic illnesses as prescribed in the physician's treatment plan based on evidence-based care and population health. Facilitate team meetings with physicians and clinical staff to discuss high risk patients; enhance the role of the medical assistant and other clinical staff via team-based services and developing initiatives to organize and maintain optimal patient care quality. Job Qualifications:

Qualifications: Education:

High school diploma, medical assistant diploma from an accredited vocational institution, or a community college course in medical assisting. Current medical assistant certificate preferred. Experience:

Minimum 3 years of recent experience working in a medical facility as a medical assistant preferred. Primary Care Office experience preferred. Other Skills:

Microsoft Office experience required. Current health records with the appropriate immunizations to work in the health care field (hepatitis B and tuberculosis). Knowledge:

Knowledge of health care field and medical office protocols/procedures. Knowledge of specific assisting tasks related to particular medical practice. Knowledge of information that must be conveyed to patients and families. Skills:

Skill in tact and diplomacy in interpersonal interactions. Skill in understanding patient education needs by effectively sharing information with patients and families. Abilities:

Ability to learn and retain information regarding patient care procedures. Ability to project a pleasant and professional image. Ability to plan, prioritize, and complete delegated tasks. Ability to demonstrate compassion and caring in dealing with others.
