IAAU is a 23 trolley bedded unit that cares for patients requiring brief hospitalization for a range of medical conditions. We have an exciting opportunity for you to join us in one of our roles as a care navigator. This role is crucial to delivering safe patient care and flow throughout the system. You must be able to work both independently and as a member of the multi-disciplinary team. The role will include supporting the shift leader to deliver effective board rounds, working with the clinical team to deliver patient care. The successful applicant will be dynamic and forward thinking; this is a fast-paced area, therefore, the successful applicant will have stamina and resilience. You will be working closely with short stay areas and the Emergency Department within the Emergency and Acute care group. You will also hold excellent communication skills and be able to articulate conversation to a wide range of expert clinicians and health care professionals. Main duties of the job

The patient care navigator will provide support to the medical and nursing team in dealing with general administrative tasks in order to free up valuable nursing and medical time to avoid delays and improve the patient experience. You will play a crucial role in helping people to get the right support at the right time and help manage a wide range of needs. This may include support with long term conditions, help with finances, and signposting to a range of statutory and voluntary sector services. The patient care navigator will be expected to work as part of the ward team under the direction of the shift leader, to ensure the smooth flow of patients through the ward. It is expected that by coordinating administrative tasks and liaising with other departments, the post holder will contribute to the efficient and effective running of the ward resulting in a significant reduction in patient journey times.
