Job summary Clinical Lead Patient Flow - Band 7 Fixed Term - Maternity Cover Chippenham Community Hospital We are recruiting to the position of Clinical Lead based within the Wiltshire Patient Flow Hub at Chippenham Hospital for a fixed term to provide maternity leave cover. The position is available from March 2025 and any seconded position will need to be agreed with their existing line manager and HCRG before commencing. Wiltshire Health and Care services are transferring to HCRG from 1st April 2025. ** Interviews will be held on 4th February ** The role involves working to deliver on all aspects of patient flow within Wiltshire Health and Care. This includes taking and processing referrals for onward care, liaising with acute and non-acute hospitals and community-based teams. You will need to have a good knowledge of the health and social care criteria and the differing services which are available. Working knowledge of the national discharge framework pathways is also important. Main duties of the job You will be a registered clinical practitioner with experience in a relevant field. You will be able to demonstrate a good understanding of the patient journey and how community services support patients. You will have good communication skills and be able to negotiate with staff from a wide range of backgrounds. You will show attention to detail and excellent record keeping. High level digital skills are required as all work is computer-based. The Wiltshire Patient Flow Hub is a team of clinical and admin staff based at Chippenham Hospital. We work closely with our acute partners, our inpatient wards, intermediate and social care providers, and community teams. We look forward to welcoming new applicants to the team. Person Specification Qualifications Essential Educated to postgraduate level or to have equivalent knowledge and experience Management, business or clinical Qualification or working towards one Desirable Evidence of development of leadership skills (accredited) Additional Criteria Essential Access to a vehicle that can be used for business purposes and hold a full and valid driving licence.
