The post holder will be responsible for overseeing and maintaining the patient safety incident investigation process for the Trust. You will have recent experience of working in the NHS, experience of working with patient safety, quality or clinical governance with knowledge and understanding of the Patient Safety Incident Response Framework (PSIRF). Applicants must be well organised and able to prioritise workloads to meet set deadlines, whilst maintaining accuracy and attention to detail. The ability to maintain flexibility in response to change and a proactive approach to work is important; as is the ability to problem solve and maintain a positive attitude and respectful, professional manner. Applicants should possess excellent communication skills; be able to work autonomously and use their own initiative to develop ideas. Main duties of the job

Oversight of all Trust investigations and reviews conducted under the Patient Safety Incident Response Framework, monitoring and reporting performance and ensuring reviews and investigation processes are followed and completed within tight timeframes which may be internal or external. Ensure that there is a robust system in place to monitor the actions and learning from reviews and investigations conducted under the Patient Safety Incident Response Framework (PSIRF). Support the development of the Trust's governance and quality systems in relation to reviews and investigation under the PSIRF. Ensure that mechanisms for sharing the findings and recommendations of the reports with a number of key stakeholders. Ensure the mechanisms in place to contact patients and families as part of the review process are robust and monitor how these are being implemented, reporting and escalating concerns. Support the dissemination of the recommendations, the learning and informing practice changes that are required. Work with the Trust's divisions to monitor progress against recommendations/action plans. Provide advice and support to staff undertaking reviews and investigations in relation to the Trust's Patient Safety Incident Response Plan/Policies/Standard Operating Procedures. Ensure robust mechanisms to ensure compliance with the statutory Duty of Candour compliance as part of the review/investigation process. About us

Qualifications & Knowledge

Experience of working within patient safety, quality, or clinical governance. Recent experience of working in the NHS. Knowledge and understanding of the Patient Safety Incident Response Framework. System-based investigations/human factors training. Knowledge of legal frameworks including safeguarding, mental capacity act, mental health act and the care act. Experience

Advanced IT skills - proficiency with Microsoft applications including MS Teams, SharePoint and Excel. Experience of preparing and presenting performance and assurance reports. Previous line management/human resources experience. Skills & Competence

Demonstrates excellent organisational and time management skills with the ability to deliver to agreed timeframes. Develop and maintain collaborative and effective working relationships with a wide range of professional and managerial staff in the organisation.
