compassionate
Homecare Patient Coordinator with proven experience in a customer service role, specifically in dealing with
vulnerable customers. You will play a vital role in coordinating patient services, facilitating communication between patients and healthcare providers, and ensuring that patients’ needs are met with
care and efficiency. The role is based at our Sunderland office - SR2 7RR. Standard working hours are 8:30 – 16:30 with a rotating schedule of 9:30 – 17:30. You will work two weeks in the office, followed by one week working from home. Your main responsibilities will be: Answer incoming calls from patients, nurses, and general practitioners and take appropriate action for each call. Coordinate with patients, couriers and internal B. Braun departments to ensure the timely delivery of critical medical products. Daily monitoring of emails to ensure all requests are processed & fully up to date. Register new patients; including account set up and home installation. Contact general practitioners or trusts to request prescription forms on behalf of patients. Facilitate the end of service coordination, including making arrangements for the return of products from a patient’s home. Identify vulnerable patients and refer to the appropriate professional body when required. Deal with potential Safeguarding issues and ensuring that the appropriate action has been taken. What you’ll need to succeed: Proven experience in a customer service-based role dealing with vulnerable customers. Experience in complaint management. Team player with the ability to work unsupervised using own initiative. Excellent communicator with ability to adapt communication style to suit the needs of the stakeholder. Detail focused and comfortable working in a fast-paced office environment. Reliable, tolerant, patient focused and determined. Good working knowledge of Microsoft Office, i.e. Outlook, Word, Excel.
