This is a new ANP role that gives the opportunity to the successful candidate to work across 4 practices, for our PCN Enhanced access and Hub service. There is plenty of flexibility about which days or hours you work. Our core hours are Monday-Friday 8am-8pm and Saturday 8am-5pm. The successful candidate's timetable can be flexibly organised around their needs (childcare, portfolio etc) in consideration with the needs of the population. Main duties of the job

The post holder will demonstrate courageous and critical thinking and is a caring, compassionate and committed experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will commit to demonstrating critical thinking in the safe clinical decision-making process. They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. East End Health Network is a well-established PCN with more than 10 years of experience. We are NHS employer, providing NHS pension if desirable. We have an established team of GPs, Pharmacists, Physiotherapists, Mental health nurses, Paramedics, Care coordinators, social prescribers and GP Assistants employed directly by our PCN and we provide both Hub and practice-based services. We have a dedicated managerial and administrative team with excellent record of support and innovation. Job responsibilities

Key Responsibilities: Independently assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis. Clinically examine and assess patient needs from a physiological and psychological perspective. Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate). Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice. Work with patients to support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed or over-the-counter medication. Prioritise health problems and intervene appropriately in complex, urgent or emergency situations. Support patients to adopt health promotion strategies that promote healthy lifestyles. Support and manage health needs of women presenting for family planning, cervical cytology or sexual health consultation. Assess, identify and refer patients presenting with mental health needs. Implement and participate in vaccination and immunisation programmes for both adults and children. Advise, support and administer vaccinations for patients travelling abroad, where appropriate. Promote and deliver evidence-based care for patients presenting with aural conditions. Meet the needs of patients presenting for opportunistic wound care. Undertake minor surgery as appropriate to competence. Person Specification

Registered first level nurse MSc or equivalent MSc in Nurse Practitioner studies and clinical assessment. Teaching and Assessing Qualification. Clinical supervision training and experience. Extended/independent and supplementary nurse prescriber. Community Nursing specialist qualification. Long Active Contraceptive qualification. Knowledge

Advanced clinical practice skills. Non-medical prescribing guidelines. Management of patients with long-term conditions. Management of patients with complex needs. Accountability of own role and other roles in a nurse-led service. Local and national health policy. Clinical governance issues in primary care. Understanding of the Leading Change, Adding Value Framework for Nursing, Midwifery and Care Staff. Knowledge of public health issues. Able to identify determinants on health in the area. Experience

Nurse-led management of minor illness, minor ailments and injuries. Nurse-led triage. Developing and implementing protocols and clinical guidelines. Clinical Leadership. Experience of undertaking clinical audits. Research experience. Non-medical prescribing. Able to demonstrate a depth and breadth of current issues and policies within the NHS. Project management. Provide education and training to other staff and students. Able to manage conflict. Competent non-medical prescriber. Perform venepuncture. Undertake routine screening (cytology, swabs). Vaccinations and immunisations. Research qualification. Experience with minor surgery. Experience in fitting Long Acting Reversible Devices. Physical, Mental and Emotional Effort

Fitness to do the job. Able to work within manual handling policy. Ability to travel for home visits and across sites. Advanced keyboard skills. Willing to complete immunisation course as recommended by occupational health. Able to deal with exposure to distressing circumstances or highly emotional events. Able to concentrate for prolonged periods. Able to work outside routine practice hours such as extended hours or enhanced access schemes. Disclosure and Barring Service Check
