Full Time - 37.5 hrs per week
£46,000 - £51,000 dependent on experience

Mosaic Healthcare consists of 3 practices located in or near Basingstoke, serving a combined patient population of circa 60,000. We are committed to building a multi-disciplinary team and enhancing our pharmacy resources.
Job Overview
To improve patients' health outcomes and the efficiency of the primary care network (PCN) pharmacy team by providing direct, accessible, and timely medicines expertise as part of a multi-disciplinary team.
Key Responsibilities:
Work as part of a multi-disciplinary team in a patient-facing role to assess and treat patients using expert knowledge of medicines.
Be a prescriber or completing training to become a prescriber.
Manage care for patients with chronic diseases and conduct clinical medication reviews.
Provide specialist expertise in medicine use and address public health needs.
Lead on person-centered medicines optimisation and quality improvement.
Support patients in their medication management through structured reviews.
Integrate general practice with wider healthcare teams to improve patient outcomes.
Develop relationships with pharmacy professionals across PCNs.
Participate in the clinical aspects of shared care protocols and clinical research.
Receive and resolve medicine queries from patients.

Community pharmacists
Minimum Requirements:
Registered with the General Pharmaceutical Council (GPhC).
Completion of an approved 18-month training pathway or equivalent.
