Havering Liberty Primary Care Network is looking to appoint a PCN Clinical Pharmacist to join Havering Liberty, a newly established Primary Care Network. The post holder will work as part of a multi-disciplinary team in a patient-facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas. The role of a PCN Clinical Pharmacist is to be responsible for the care management of patients with chronic diseases and to undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially older people, people in care homes, those with multiple co-morbidities (frailty, COPD, and asthma), and people with learning disabilities or autism. Key Duties and Responsibilities include: To be a prescriber or completing training to become a prescriber and work with and alongside the general practice team. To undertake the care management of patients with chronic diseases and to undertake clinical medication reviews. Provide expertise in medicines optimisation across the network. Support the integration of general practice with wider healthcare teams. Managing Safety and CAS alerts. Assisting with QOF and incentive scheme audits. Attending Medicines Management Prescribing forums quarterly. CQC preparedness. Care home support, including annual medication reviews. Discharge summary reconciliation. Polypharmacy Medication reviews. Supervision and training of reception staff in issuing repeat prescriptions. Telephone advice to patients for queries on medication and minor illness. This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service. £48,000 to £54,000 a year, inclusive and dependent on experience.
