About the Role Medics PCN are looking to welcome 2 qualified and experienced clinical pharmacists into our PCN on a full-time basis - one role is for 6 months and one is for 12 months, please state in your application which role(s) you are interested in.

The successful applicant will work as part of a multi-disciplinary team in a patient-facing role. They will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.

The main responsibilities of the role include:

Provide primary support to general practice staff with regards to prescription and medication queries. Support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the GP practice(s). Provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services. Ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The post holder will be supported to develop their role to become a non-medical prescriber.

This key role within the PCN is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.

About the Candidate The post-holder will have good clinical backgrounds and possess the requisite skills required to support safe, evidence-based, clinically and cost-effective prescribing.

You will be a qualified pharmacist with strong experience within primary care and have the following skills and qualifications:

Completion of an undergraduate degree in pharmacy. Registered with the General Pharmaceutical Council. Completion of CPPE primary care pathway. Minimum of 2 years experience as a pharmacist demonstrated within a practice portfolio. Have experience and awareness of common acute and long-term conditions that are likely to be seen in general practice. About Us We are a stable and mature PCN, serving a patient population of 56,000. The Medics PCN is made up of the following collaborative practices: Woodland Avenue Surgery, Medici Medical Centre, Bell House Medical Centre, Barton Hills Medical Group and Gardenia & Marsh Farm surgery.

Medics PCN are in a privileged position to already have 4 PCN Clinical Pharmacists, who support each other to deliver the PCN DES and capacity at each of our practices.
