This is having a positive outcome for many children and their families’ About you… • Educated to NVQ L3 • Good motivational skills and experience of supporting Children in care planning processes and development of their independence skills • Knowledge and experience of working with Care Leavers and able to provide support, advice and guidance in education, employment, training opportunities • Significant understanding of pathway planning with Care Leavers • Experience of using ASDAN programme to enable Care Leaver to grow their independence skills • Understand immigration legislation and able to support our Unaccompanied Asylum-Seeking Children and Care Leavers About the role… To ensure young people have the right support, at the right time for them to transition into adulthood and leave care. Duties will include: • Assisting Social Workers and others in the implementation of assessment, intervention and pathway plans • To consult with care leavers in service planning and support them to develop their skills and confidence to make representations in various forums • Supporting care leavers with housing and other needs • To keep accurate records on young people and update information systems • To assist in the production of reports as required The successful applicant for this post will be subject to an enhanced check by the Disclosure and Barring Service (DBS). The costs for the DBS are paid by us. Please note you will need to have a driving licence to enable you to travel around the county. The Personal Advisor to Care Leavers interviews will be held on 16 January 2025. Please be advised this could be subject to change, in which case we will give appropriate notice.
