Job Description - Personal Care Assistant (PCA), Heritage at Falmouth (250121-50022715) Purpose of Position:

Assist residents in activities of daily living and intervention as appropriate to meet the needs of the participants. Is knowledgeable of and understands the needs of the resident population. Description:

Is knowledgeable of and practices departmental procedures, safety regulations, and adheres to local, state, and federal guidelines and follows policies that will effectively maintain programs and operations for Heritage. Assists residents in activities of daily living including dressing, toileting, bathing, grooming, and oral hygiene in a professional manner, following plans of service for residents set by the management team. Makes accurate entries and charts according to Heritage policy and procedure. Strictly follows SAMM policies and procedures. Adheres to Universal Precautions and Infection Control Policies, including following established hand washing procedures, use of appropriate PPE and supplies when handling infectious materials, and follows established policies governing the use/disposal of PPE and disposal of infectious wastes. Has a competent understanding of all chemicals and has been trained to use them in accordance with standards. Immediately reports all accidents/incidents to your supervisor. Wears and/or uses safety equipment and supplies and uses proper mechanics while performing duties (e.g., back brace, mechanical lifts, etc. when moving or lifting heavy objects). Adheres to department working hours and completes assignments within guidelines set by supervisor. Demonstrates versatility and/or willingness to cover various functions, shifts, driving, working weekends, holidays, during emergencies, and in other positions when necessary. Drives Heritage vehicle for errands, transporting, or other uses as directed. Develops and maintains a good working rapport with inter-departmental personnel, and other departments within Heritage, to assure that programs can be properly planned and maintained to meet the needs of Heritage. Participates in programs designed for in-service education, on-the-job training, and orientation classes. Maintains the confidentiality of all resident care information, including protected health information. Performs other job-related duties as requested by the Executive Director and/or designee to meet the needs of the residents and the facility. Consistently provides service excellence to all residents, visitors, volunteers, and co-workers in a manner that reflects Heritage at Falmouth’s commitment to CARES: compassion, accountability, respect, service, and excellence. Qualifications:

High School degree Must be certified CNA or HHA or Completion of 54 hours Personal Care Training Must possess basic computer, telephone, and radio skills. Must be able to speak the English language in an understandable manner. Must be able to lift a minimum of 50 pounds. Must have a current MA Drivers License Schedule Details:

40 hrs. per week, Rot Days, 6:30a - 3p., E/O Weekends & Rot. Holidays. *Sign On Bonus Eligible* Organization
