To help achieve this the Preparing for Adult Life (PfAL) Team has been set up as Norfolk’s transition to adult social services team. We are looking for an enthusiastic PfAL Assistant Practitioner who will have the ability to work with young people from age 17 up to 20, as well as their families, be able to demonstrate an understanding of the legislative requirements and how to support young people to develop their independence. The role will involve carrying out assessments and care and support planning under the Care Act 2014. You will also have a commitment to working in partnership with families as well as with the range of professionals and other agencies who could also be involved in supporting the young person as part of preparing for adult life.
