Chelmsford City Health PCN Limited has an exciting opportunity for a hardworking, enthusiastic and innovative pharmacist to join our primary care network. This role is offered on a full-time basis, for 37.5 hours per week; however, part-time working will be considered. Main duties of the job

To support the effective delivery of patient care as part of the PCNs medicines management team and alongside the wider surgery teams. The post holder will be: A senior clinical pharmacist who will take responsibility to proactively manage patients with complex, long-term conditions such as hypertension, diabetes and COPD, ensuring that medication is optimised, titrated, monitored and complied with; Providing pharmaceutical medication review and health education advice to patients, by face-to-face, telephone or notes-based review, as agreed with the practice/PCN; Providing clinical advice and expertise on the repeat prescribing re-authorisation process to surgery staff by supporting repeat prescription systems, dealing with acute script requests and medication reviews; Carrying out and providing clinical leadership on medicines optimisation and quality improvement; Delivering on and managing relevant aspects of the PCNs Directed Enhanced Services, Quality Outcomes Framework and Investment and Impact tasks; Acting as a key contact for all medication related queries; Liaising with other providers i.e., secondary care, community pharmacy and care/nursing homes as necessary to ensure safe transfers of care; Supporting improvements in clinical care, through practice-based audits and implementing change; Supporting the PCN clinical directors, as required. Experience

Awareness of the breadth of common acute and long-term conditions that are likely to be seen in a general medical practice; Ability to demonstrate general pharmaceutical skills and knowledge in core areas; Ability to plan, manage, monitor, advise and review general care plans for patients with long-term conditions; Ability to demonstrate accountability for delivering professional expertise and direct service provision as an individual; Ability to recognise priorities when problem-solving and identify deviations from normal patterns, referring to seniors or GPs with confidence, when appropriate; Ability to follow legal, ethical, professional and organisational policies, procedures and codes of conduct; Ability to assist patients in decisions about prescribed medicines and supporting adherence as per NICE guidance; Non-judgemental attitude and ability to be flexible and adaptable; Self-motivated with excellent communication skills; Understand the need for patient confidentiality. Qualifications

An undergraduate master's degree or equivalent in Pharmacy. Membership as practising pharmacist with The General Pharmaceutical Council. Commitment towards continuing professional development in accordance with requirements of The General Pharmaceutical Council. Pharmacist independent prescriber registered with The General Pharmaceutical Council. Professional indemnity insurance is required and will be reviewed on an annual basis. Post-qualification experience in pharmacy (hospital, primary care or community), or working towards obtaining this; Member of the Royal Pharmaceutical Society (RPS). Disclosure and Barring Service Check

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. £50,000 a year in the region of, dependent on experience.
