Job Summary:

As a Pharmacy Technician in our General Practice, you will play a vital role in supporting the Medicine Management team and ensuring patients receive the best possible care. This position combines patient-facing responsibilities with analytical and administrative tasks, contributing to the optimisation of medication use and the improvement of prescribing processes.

Main duties of the job Key Responsibilities:

Medication Reconciliation: Review and reconcile medications from hospital discharge letters to ensure continuity of care.

Audits and Drug Monitoring: Conduct regular audits and monitor drug usage to ensure compliance with best practices and safety standards.

Repeat Prescription Management: Handle repeat prescription issues and queries, promoting efficient processes such as repeat dispensing and online ordering.

Medication Reviews: Assist in conducting medication reviews to optimise patient outcomes and minimise clinical risks.

Collaboration: Work closely with the Medicine Management team to implement practice improvements and prescribing initiatives.

Patient Interaction: Communicate effectively with patients to provide guidance on medication use and address any concerns or queries.

Professional Development: Engage in continuous learning and development opportunities, supported by regular access to a GP and participation in team meetings.

There is ample free parking on site and the sea is a 5 minute walk from the practice.

We also like to have fun and routinely have activities at lunchtime for staff. We recently repurposed the terrace at the front of the building so staff can sit outside in the summer.

Job responsibilities

Stream general prescription requests and work with the reception teams, so as to allow GPs and clinical pharmacists to review the more clinically complex requests.

Assist patients with responses to medication queries.

Provide training and support on the legal, safe and secure handling of medicines, including the implementation and increased use of the Electronic Prescription Service (EPS).

Promote Electronic Repeat Dispensing (eRD) and online ordering as appropriate to patient and practice needs.

Implement efficient ordering and return processes and reduce medication wastage, liaising with patients as needed.

Support the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings.

Help to organise and run various systems for the delivery of QOF and locally commissioned enhanced services.

Review and develop policies in line with CQC compliance.

Develop relationships with other pharmacy professionals and members of the multi-disciplinary team including primary care, community pharmacy, secondary care and mental health.

Contribute to the delivery of effective daily processes for the transfer of care between hospitals and GPs including effective signposting, record updates and related communication.

Through the use of audit and action planning, help to tackle health inequalities.

Person Specification Experience

Experience of working in Primary Care.

Professional registration with GPhC.

Good clinical pharmacy knowledge including terminology.

Computer literate (MS office packages).

Excellent communication skills with ability to adjust to suit the audience.

Strong understanding of data protection and confidentiality.

Self-motivated and pro-active.

Experience of using SystmOne or Emis.

Qualifications

BTEC/NVQ level 3 or equivalent in pharmaceutical sciences.

Be on the Primary Care Pharmacy Education Pathway.
