Undertake medicines reconciliation in accordance with practice protocols and to update patients medical records/prescribing accordingly. Carry out medicines optimization tasks including effective medicine administration and technical medication reviews. Utilize consultation skills to work in partnership with patients to ensure they use their medicines effectively. Maintain full and complete records of all patient contacts using appropriate clinical templates and coding. Support medication reviews and medicines reconciliation for new care home patients. Support the achievement of the practices prescribing Quality and Outcomes Framework targets. Working with the dispensary manager/clinical pharmacist, contribute to the review and development of repeat prescribing protocols/standard operating procedures to improve the efficiency and effectiveness of repeat prescribing systems. Experience
Experience working within primary care. Understanding of prescribing and medicines management issues. Evidence of continuing professional development. Ability to use own initiative, discretion, and sensitivity. Ability to communicate complex and sensitive information effectively with people at all levels by telephone, email, and face to face. Experience in either a community pharmacy setting. Experience of prescribing systems such as SystmOne. Excellent organizational and planning skills. Knowledge of IT systems including the ability to use word processing skills, emails, and the internet to create simple plans and reports. Qualifications
