Pharmacy Technicians play an important role in Primary Care, complementing clinical and community pharmacists and other members of the practice multidisciplinary team.
The purpose of this role is to lead improvements to maximise safe, cost-effective best practice in prescribing to improve patient care. The role involves reviewing and recording the medication the patients should be taking, ensuring sufficient supplies, administering medicines, and counselling patients to support their understanding on how to use their medicines safely.
Optimising and rationalising medication, switching medications to agreed and approved protocols, improving repeat prescribing processes, including promotion of repeat dispensing, online ordering, minimising clinical risk and aiming to reduce waste in medicines.
The individual will support medication changes from secondary care or community specialists in line with the practice policies and actioning any medication changes that are required after reviewing hospital discharge and clinic letters.
This is a part/full time vacancy - minimum 27 hours per week.
Main duties of the job
Work with the Clinical Pharmacist, Community Pharmacists and Clinicians in accordance with agreed practice prescribing standard operating procedures.
Process repeat medication requests, including the conversion of acute requests to repeat medicines, changing quantities of medicines.
Issuing prescriptions in line with agreed clinical protocols and within your competency.
Regular liaison with community and acute hospital pharmacists.
Provide advice and guidance to patients regarding medication optimisation and lifestyle inhaler techniques.
To provide medication review services to patients via clinics in the practice.
To support the achievement of the practices prescribing Quality and Outcomes Framework targets.
Dealing with medication queries including acute medication requests, patient medication and prescription queries.
To update and maintain accurate patient medication records on the practice clinical computer system, including advice given and action taken.
Liaise with secondary and primary care colleagues to ensure correct medicines are continued following the transfer of care.

Job responsibilities
To work as part of the prescribing team providing support for the practice team (both clinical and administrative).
To provide medication review services to patients via clinics in the practice.
Help support the dispensary team by running DRUMS on the specified cohort of patients.
Help support the practice with recalls.
To support the achievement of the practices prescribing Quality and Outcomes Framework targets.
To assist the clinical and management team with data analysis of prescribing behaviour in line with NICE guidance and local clinical commissioning directives.
Improving repeat prescribing processes, including promotion of repeat dispensing, online ordering, minimising clinical risk and aiming to reduce waste in medicines.
To support the Prescribing team by sharing appropriate medication guidance and establishing a method of communication to both administrative and clinical staff.
Assist in the development of patient information leaflets.
To assist with the development and review of medicines audits.
To maintain and develop medicines communication to staff.
To support staff in the promotion of healthy lifestyles and support those with chronic diseases to develop good lifestyle changes to assist in the management of their conditions.
To assist in the development of relationships with secondary care, community specialists and community pharmacists.
To support medication changes from secondary care or community specialists in line with the practice policies.
Actioning any medication changes that are required after reviewing hospital discharge and clinic letters.
Provide a contact for patients, secondary care and community pharmacists with medication queries.
Lead on project work/prescribing initiatives/clinical research under the support of the Clinical Pharmacist or Clinical Lead.
Help run and keep up to date with the relevant CQC searches within the practice.
Please note you must be registered with the General Pharmaceutical Council as a Pharmacy Technician and hold a BTEC/NVQ Level 3 in Pharmacy services or equivalent Pharmacy Tech Qualification for this position.

Educated to A-level/equivalent or higher or equivalent demonstrable experience
GCSE English (C or above) and at least three others
Qualified Dispenser (NVQ Level 2)
