Job responsibilities

We are looking for a confident Physical Health Lead Nurse (RGN) who can inspire, support and oversee the delivery of the very best care.

You'll be working 24 hours a week (9am - 5pm but can be flexible).

You will be based at Cygnet Hospital Sheffield but will require to support Cygnet Oaks.

Cygnet Hospital Sheffield offers a low secure service for women and CAMHS services for male and female adolescents over three distinct wards.

Within the adult service, women are supported by a full multi-disciplinary team who are focused on helping service users regain independence, maintain family links and prepare for discharge. There is a focus on community access, therapy and meaningful activity and offering a warm and welcoming environment for the delivery of support and care.

Your day-to-day

The Post Holder will be responsible for monitoring the physical health of patients across two sites Cygnet Hospital Sheffield and Cygnet Oaks, working closely with the services multi-disciplinary teams. The role will involve co-ordinating the GP appointments on a weekly basis, taking responsibility for referring patients if other health professional involvement is indicated, ensuring that documentation is completed i.e. GP notes, physical health monitoring forms and Physical Health Care Plans. The role will involve liaising with the teams on each clinical area, responding to the educational needs of patients and becoming involved in the training of staff as required. The post holder will monitor the SLA contract (alongside the Hospital Manager) with the local GP surgery and will develop partnerships with specialist professionals in primary and secondary care to achieve holistic care provisions. The post holder will manage the physical health support workers within the team and support the development of their clinical skills. The post holder will take the lead on Health Promotion Groups/1:1 interventions and initiatives across the hospital with support from the Quality Lead and other MDT professionals. CQUIN SITES: You will be responsible for the coordination and monitoring of the CQUINS for Physical Health and to achieve sustained improvement in the physical health key performance indicators.

You An experienced RGN with a current Pin Number, you'll have

Essential: At least two years experience as a Registered General Nurse. The ability to maintain documentation, undertake audits & demonstrate integrity in your role. A good understanding of the latest nursing practices. A commitment to patient recovery in order to effectively monitor, manage & reduce risk. Compassion, resilience & a dedication to empower & support service user independence.

Desirable: An interest in leadership, research & assessment.
