Overview of the Role: As a Physical Therapist on our client's Home Health team, you'll play a crucial role in enhancing patients' lives by providing essential Physical Therapy services straight to their homes. Your work will involve assessing patient functioning levels, crafting personalized treatment programs, and developing strategies to boost independence and improve physical and cognitive abilities. Additionally, you'll contribute to educating patients and caregivers, ensuring they are well-informed and supported throughout the therapy process. Responsibilities:

Conduct comprehensive Physical Therapy assessments and develop effective treatment plans. Document and communicate patient responses to therapy and any changes in their condition. Educate patients and caregivers on therapy regimens and proper use of therapeutic devices. Oversee the work of physical therapy assistants and, when necessary, supervise certified nursing assistants. Compile detailed reports and participate in professional development and agency activities. Requirements:

Valid licensure as a Physical Therapist. Minimum 1 year of related experience.

Proven ability to assess and create individualized treatment plans. Strong communication skills to effectively instruct patients and caregivers. Ability to supervise support staff. Dedication to continuous learning and professional growth. Benefits & Perks:
