We make a difference to peoples lives and the community by working with people to rehabilitate. This is person centred and takes place in the persons home, so why join us? We are a supportive multidisciplinary team working out of a shared office space in the centre of Barnstaple. The Physiotherapists and Occupational Therapists work together with support team members as one therapy team. We work in collaboration with the social care and community nursing teams along with the coordinator and admin team members.

Role Summary - A typical day as a community physiotherapist

You will be based at Barnstaple Health Centre, co-located with social care and community nursing promoting a timely and successful journey for our shared patients/clients.

A typical day involves catching up with the wider team, planning and attending your community visits, as per the clients requirements, some will be lone working, others will be joint visits either with therapy or the wider team.

We work with people who are registered with one of our 4 local GP surgeries Litchdon, Queens, Brannams and Fremington Medical centres. We visit patients in Barnstaple town, in the surrounding villages and in rural areas.

My Care is our secure electronic healthcare record system, this allows you to record live at the point of seeing the patient using your NHS mobile phone as well as on your laptop back in the office. You will be provided with a mobile phone and a laptop for your work.

Your daily routine will involve regular communication with other health and social professionals and wider partners including Millbrook our equipment provider, the Independent Living Centre, Care Homes Team, GPs, the Community Response and Recovery Team and One Barnstaple (voluntary sector).

Our shared role being to support our patients/clients and their presenting needs safely within their own home/environment.

Do you have an interest in rehabilitation? We can offer you experience in a range of conditions orthopaedic, amputees, neurological, mental health and physical health.

As a community Physiotherapist we work closely with patients to enable them to be active members of the community and we offer high quality care to support their needs.

Our main objective as part of a community multidisciplinary team is to provide education and intervention to allow patients to have tools to self-manage. We work in partnership with our patients to set goals on what they want to be able to achieve. We work with people to avoid a hospital admission and we support discharges from hospital.

We have a wealth of community groups and support in the Barnstaple area including exercise groups, chair-based yoga, healthy lifestyle support groups and support for people to manage tasks in and around their home.

As a specialist clinician you will have the opportunity to mentor and supervise other clinicians, to reflect, to contribute towards your CPD portfolio and lead on student education.

Working Pattern:

This community therapy role is for 37.5 hours per week Monday to Friday with additional on-call and weekend working responsibilities.
